• Active Subscription Product (e.g., Month to Month, 3 Months, 6 Months, etc.)
• Active Subscription Start Date
• Amount Paid for Active Subscription
• Paypal Account email (if you have one)
• Mailing Address (required only if you do not have a Paypal account)

Prorated refunds will be made using Paypal or by check (via mail) once we verify your request is accurate and complete.
